The following are fillable forms in Excel and most can be completed on your computer or printed blank and filled in by hand. Save them to your hard drive as a master copy, then open and "Save As" a new filename for each new period. That way, the master will always be clean and blank for you to use again. After completion, you will need to print out, sign, and forward to your supervisor for signature.
